This problem is definately happening a lot more frequently in 3.1, and I'm fairly sure that the blip dissapears when the associated player respawns. I think perhaps the code that tracks the blip doesn't ignore dead players.
Treat this as a placeholder report until we've had time to analyse the problem further.

Get an icon over you head, like the a request for welding, then use a phasegate to go somewhere else. The icon over you head will be left behind, and it could brighten up the map.
